Historically, queer has been a synonym for gay. However, recently it has been expanded to mean any kind of non-"normal" sexuality, physical identity, or gender identity.

People who are into BDSM may consider themselves queer. People who are asexual may consider themselves queer. Transexuals who self-identify as straight are certainly queer, insofar as queer encompasses anything within the LGBT acronym.

Queer originally meant "weird, odd, or different" in any respect, so the recent shift brings it closer to the original meaning.
