Some trans-friendly organizations (medical providers, employers, etc) will record both someone's legal name+gender and their preferred name+gender (but make a clear distinction between them).

I find this very interesting. Many trans people wish that others wouldn't use their legal name/pronoun ever, and ONLY use their preferred name/pronoun. However, it's a practical reality that your legal name+gender is sometimes required for government identification purposes. When organizations clearly state that "we will use your preferred name whenever possible, BUT we still need to know your legal name for a few limited purposes", they are being very proactive in recognizing trans rights, while still accomodating the (somewhat broken) system as it is.
