To format some media as bootable:

at a BARE MINIMUM, copy these three files over:
- command.com
- io.sys
- msdos.sys
write boot-code to the first sector of the partition. There are several ways to do this:
- mkbt (described some more here)
- ms-sys (included in Knoppix)
- cfdisk (usage notes here)
